• Personal Identification Details: Full name, contact phone number, email address, national identity card (NIC) or passport number.
• Appointment & Patient Records: Doctor preferences, appointment dates, consultation history, and family member details added for channeling.
• Payment Transaction Data: Transaction IDs, payment statuses, and billing receipts. (Sensitive card details are submitted directly to PayHere payment gateway and are not stored by SLH).

2. How We Use Your Information

• Service Delivery: Facilitating doctor channeling and hospital appointment bookings.
• Healthcare Provision: Sharing necessary appointment details with Southern Lanka Hospitals medical staff.
• Payment Processing: Sharing user names and billing info with PCI-DSS compliant payment providers (PayHere) for secure payment processing.
• Notifications: Sending appointment confirmation reminders, health notifications, and service updates.

3. Data Sharing & Third-Party Gateways

We do not sell, rent, or trade user personal data. Information is shared strictly under the following conditions:

• With Southern Lanka Hospitals: shared with hospital administration and treating doctors for healthcare provision.
• With Payment Providers: Name and transaction amount shared securely with certified payment gateway partners (PayHere) to execute payment transactions.
• Legal Compliance: When required by law, regulation, or legal proceedings in Sri Lanka.

4. Data Retention, Security & Deletion

• Retention: Account details and transaction history are retained while your account remains active.
• Deletion Requests: Users may request account deletion by emailing app@slh.lk. Personal data no longer required will be deleted or anonymized, except where retention is required for statutory, financial record-keeping, or legal compliance reasons.
• Security Safeguards: We implement industry-standard encryption, firewalls, and access control protocols to protect user data.
